I, the undersigned, hereby acknowledge and accept that participating in the Great Eastern Mall Picklefest 3.0 involves inherent risks, including but not limited to physical injury, accident, or illness. I agree to participate at my own risk and release the organisers, sponsors, partners, officials, and volunteers from any liability for loss, damage, injury, or death, except where caused by proven negligence. I confirm that I am physically fit to participate, will abide by all tournament rules and official instructions, and consent to the use of my name, photographs, and videos taken during the event for promotional purposes without compensation. I understand that the organiser reserves the right to amend event details due to weather, safety concerns, or other unforeseen circumstances.

By submitting your details, you agree that your personal data may be collected, used, and processed by the organiser (Great Eastern Life Assurance (M) Berhad & Great Eastern Mall) for purposes related to this event, including registration, communication, scheduling, results, marketing updates, and future event information.

Liability Waiver & Informed Consent

I acknowledge that pickleball involves risks of serious bodily injury, including permanent disability, paralysis, and death. I agree to release and hold harmless the organizer and its agents, volunteers, committee members, sponsors and affiliates from liability for bodily injury, property damage or death arising from my participation in activities of the club program. I give this release and indemnification in exchange for the opportunity for me to participate in the tournament.

Media Release

I grant permission to the event organizers to use photographs, videos, and recordings of me taken during the event for promotional purposes, without compensation.

Personal Data Protection Act (PDPA) Consent

I acknowledge and agree that the organizer may collect, store, and process my personal data for this event and future communications related to pickleball tournaments. The tournament organizers will collect and process my personal details such as full name, contact details, email address, DUPR rating, and emergency contacts in accordance with the Personal Data Protection Act 2010. My personal data will not be sold or shared with third parties beyond these stated purposes.
